Pursuant to Application for permit to construct along a stream and/or water quality
certication.

In accordance with KAR 4:060 Section 3,
notice is hereby given that the Letcher County
Fiscal Court, 156 Main Street Suite 107,
Whitesburg Kentucky has applied to construct
along The North Fork of the Kentucky River.
The site is located at the mouth of Kingdom
Come Creek. The purpose of the work /
construction will be to remove debris from
the base of the bridge pier and stream bed
to protect the integrity of the bridge structure
and to relieve any stress on said bridge during
periods of high water.

The proposed site is located on the Roxana
U.S.G.S 7.5 Quad Map, Located 1.1 miles
east from the intersection of Highway 160 and
Highway 588 at Premium Kentucky.

The permit application has/will be lled for
public inspection at the Department for Environmental Protection, Division of Water,
Floodplain Management Section, 300 Sower
Boulevard, Frankfort, Kentucky KY 40601.

The proposed site is located on the Mayking
U.S.G.S 7.5 Quad Map, Located 2.6 miles
northeast from the intersection of Highway
15 and Highway 7 North at Isom Kentucky.

southeast from the intersection of Highway 7

PUBLIC NOTICE OF
BLASTING SCHEDULE

Notification is hereby provided that Nally & Hamilton Enterprises,
Inc., P.O. Box 157, Bardstown, KY 40004, (502) 348-0084, Permit
No. 867-0552 will be blasting at Meadow Branch, near Eolia, Latitude 37° 04’ 04” , Longitude 82° 44’ 57”. Blasting will be performed
daily, Monday through Sunday. No blasting will be conducted before
sunrise or after sunset except in accordance with 405 KAR 16:120
Section 4. At least 10 minutes before the blast, access to the blast
area will be controlled by using company personnel as flagmen.
Traffic will be stopped on all public, access and internal roads a
minimum distance of 1000 feet from the blast area. Before each
blast is detonated the following type(s) of audible warning will be
given: One (1) minute series of long blasts five (5) minutes prior
to the blast signal. The blast signal will be: A series of short blasts
one (1) minute prior to the shot. The all-clear signal(s) after blasting will be: A prolonged blast following the inspection of the blast
area. The device to be used for all audible warnings is a siren that
is audible for a minimum distance of 0.5 miles from the point of the
blast. Blasting will not be conducted at times different from those
given above except in emergency situations where rain, lightning,
other atmospheric conditions, or operator or public safety requires
unscheduled detonation. Prior to these detonations, the above
listed audible warning will be given. Also, prior to detonations in
emergency situations the permittee, using audible signals, shall
notify all persons within one-half (1/2) mile of the blasting site.
Any unscheduled blast will be recorded in the blasting record and
reported with the cabinet not later than three (3) days after the
unscheduled blasting, not including Saturday’s, Sunday’s or legal
holiday. The report shall include all of the information required by
405 KAR 16:120 Section 4 (1) (b) 3. The blasting record is to be
kept on file for at least five (5) years.
